The transition from foster care to adulthood can bring significant changes and new responsibilities. Mathern Supervised Independent Living provides young adults with a safe, structured environment where they can develop practical skills, work toward personal goals, and build greater confidence in managing everyday life.
Our individualized support is designed for young adults ages 18 to 21 participating in extended foster care and supervised independent living programs.
Independence does not mean doing everything alone. The right support can give young adults the opportunity to practice new skills, make decisions, learn from challenges, and gradually take greater responsibility.
Our team provides structured residential support alongside individualized coaching, supervision, coordination, and progress monitoring.
Residents receive support in a safe and structured residential setting that encourages consistency, responsibility, personal growth, and increasing independence.
We help monitor progress toward important goals related to eligibility, education, employment, appointments, transition planning, and other individualized objectives.
Residents receive practical coaching in budgeting, household routines, organization, community access, problem-solving, decision-making, and other skills that support successful independent living.
Daily routines create stability and confidence. Residents receive support with household responsibilities, organization, schedules, task completion, and routines that encourage accountability.
We support residents as they navigate their communities, access resources, attend appointments, complete errands, and develop greater confidence managing everyday activities.
Residents may receive medication reminders and related documentation as permitted within their approved services and support plan.
We support appropriate communication and coordination with authorized members of the individual’s care team. Progress, services, and relevant concerns are documented according to established procedures.
Our support may include supervision, safety planning, behavioral observation, incident reporting, and communication regarding safety or behavioral concerns.
Additional sitter support may be available when authorized based on the individual’s approved support needs.
Mathern Supervised Independent Living is designed to create opportunities for young adults to practice independence in a supportive environment.
Residents can work toward meaningful goals, strengthen everyday skills, develop greater responsibility, and prepare for the next stage of adulthood with support along the way.
Every step toward independence matters. Through structure, encouragement, practical coaching, and individualized support, we help young adults develop the skills and confidence needed to move forward.
